Complete bug fixes for AquilaCRS v2.2

• ADJ_LOS & ADJ_LOS_GRP must output NULL in the event of an invalid input.
• Expose the FixUnlinkedDependencies method on the MetaService.
• IBID v2.2 - New Fields
• CPAX Assessment (Dependency)
• Add the CALCULATION_VERSION and LAST_CALCULATED field from DEP & IBID Analysis to the FDS & KPI reports for Ken
• Thread Error on Software Upgrade poss not restarting the Replication Timer.
• ICU Historic Data needs to be available for imported data, AFTER the 1st Jan 2014.
• Redesign the internal code for caching data structures for all Query items.
• Open Folder and Open Patient functions are available in Query Results when there is no data (i,e query not yet run)
• Dependency Form: Delete function not working with v2.0 server - gives `Please upgrade` message
• Patient Demographics : Delete function not working with v2.0 server.
• Patient Folder: Delete function not working with v2.0 server.
• Tools > Options > System > Upgrade button shows `TO DO` message.
• `Pre Injury` Psych & Physio scores required. This is a one-off assessment record and so should be standalone from rest of Dependency record.
• Psych & Physio Scores: In pre-injury mode the Assessment date/time MUST be visible, but NOT as a standard dependency assessment
• Make login screen controls more compact.
• Dep ICU Scoring - width of checkboxes increases causing extra space in columns & making usage with small screen sizes difficult.
• IBID Query - Next of Kin relationship field has a spelling error - `Nexct of Kin .... `
• The IBID Delete function on the server must be updated to delete the DEP_SCORES record for the Pre-Injury data
• Field **Next of Kin Relationship Code** is (incorrectly) showing in the IBID Query & has spelling error.
• In touch mode, the navigation buttons on the dashboard, patient folder and ibid screen don't resize.
• Partial Regression Test
• Pre-Inj Psych & Physio Scores tab not appearing in Dependency form if user is already on that tab
• Incorrect splitter position on the dashboard screen

• Care Journal Update
• #analysis Admission field calculation needs to take into account the Death value if patient dies on day 1.
• #BUG BSA_GRP field has a space in the `_0 %` value (between the 0 and %) that needs removing.
• Decision Support field not outputting any data. Field is present in the query definition, but not data showing in the column in query results.
• VIEW_IBID_MAIN needs rebuilding to cure Place_Of_Birth_desc field lookup error.
• Significant Event: Staff Name drop down list is not populated with Staff Names.
• Significant Event: end date is allowed to be before start date, and is therefore never shown on the journal.
• Care Journal: Date Navigator Issues
• Care Journal: automatically show event details when clicking on an event.
• Care Journal: Event Details pane - the rows of details are not in a logical order.
• Add option to show 'Daily' dependencies on Care Journal
• IBID_BC_UK_2010 codelist: rename value `Burn Facility/Community` to `Burn Sub-Facility`.
• Care Journal : View - Time Line not working (causes app to hang)

• Alter server install script to allow IntegratedSecurity option instead of user/password combo
• New Audit Trail Status: 2 (Won't Replicate)
• Update the Audit Trail code to ensure logging the correct SQL tablename.
• Causation > Add Injury Date/Time Accuracy field

• On the IBID dashboard, make the Recalc Analysis function a sub-item of an `options` button, so that it's less easy to press accidentally.
• BUG - AquilaServer LogChange2DB seems to be getting the wrong date for the LAST_UPDATE_TIME field
• Making actions visible/enabled or not based on user privileges (updated)
• Make Date edit drop downs touch-friendly.
• Query Results must be able to open the correct data-entry form for the item being queried for, and not just open the patient folder.
• Physiotherapy>CPAX fields shown
• Central Cyanosis - make field hidden if the rest of the Inhalation symptoms are hidden
• Add `Critical` level to validation rules.
• Dependency - the drop down of available IBID records does not show the Record Type, only the Trauma Network ID.
• BUg: Acces violation opening ANY page on newly created Dependency after accessing the Intervention/Tx page.
• Bug: Dependency - setting/removing the Record Date/Time whilst on another tab does not update the validation messages associated with it (thus you still can't save & close the form)
• `Invalid column name 'TBSA_DATASOURCE'.` on Burn Injury page
• New Dependency from within a New IBID = no linked IBID record.
• Bug: filter property cannot be used on detail tables.
• Dep ICU Scoring. Alter the star markings to match the Organ Support calculation `feeder` fields.
• Add `New Dependency` button to the `mini-toolbar` on the IBID section of the patient folder (where it used to be)
• Add `New Significant Event` button to the Patient Folder navigation bar.
• BUG Dependency Theatre page - no validation messages showing - should be at least one for GRAFTED_TBSA
• Increase number of columns in IBID List
• IBID - Phsyio & Psych Pre-Injury Scoring page is not showing the correct Title (i.e. it's not saying `Pre-Inj`
• Bug: List of IBID reccord on Patient Folder home page shows `Source of Injury` as `SOURCEOFINJURY_desc`
• Some server calculated fields are not showing any data in the IBID Summary. E.g. Time Period Post Injury & Age Now
• Change `Intensive Care` tab to `Critical Care` tab in Dependency.
• User Interface Re-organisation
• Bug: `Open Folder` from Dependency screen faulty. Attempts to create a new folder for same patient.
• Bug: `New Dependency` action from patient folder, opens `IBID record selector` but does *not* highlight the currently selected IBID record.
• Bug: Creating new dependency: Pre Injury Psych/Phisio is not displayed until the dependency has been saved & closed.
• Dependency/IBID Calculation Inaccuracies
• Changing the Date in Daily Dependency clears the fields ticked in Intensive care/Critical Care dataset.
• Dependency Changes to ensure Dependencies are always linked to an IBID record
• Add scoring advice to AusTOMS section in Dependency.
• Server upgrade/Database upgrade to v39: some validation messages for Dependency Record Date/Time are incorrectly updated to `critical`.

• Remove `Database Upgrade` from the startup procedure (i.e. after login)

• Remove Xigris Drotrecogin Alpha from the iBID Record Contents report and all Dependency related queries
• Source of Injury not appearing in the IBID card view
• #analysis The result WA from the Admission field should be renamed WA/DC.
• Max Records dialog. Auto tick the box so the user doesn't have to.
• BUG: `Invalid column name 'CALCULATION_VERSION_1'.` when running IBID-Dependency Query with this field in the output.
• Principle Inhalation Agent (Airway Injury) appears twice in the iBID & Dependency Query
• Print Preview on Patient Demographics not working correctly.
• Possible duplicate records bug in Dependency - now fixed but need to remove any duplicates in the database during 2.2 upgrade.
• Some new v2.2 fields are still visible for record types they shouldn't be.
• Add FRAILTY_SCORE field to queries
• When Tools > Options needs to restart, a) change the button caption to Restart, and b) give the user another close & restart later option?
• Dependency: Linked IBID Record drop down NOT WORKING
• Significant Events: change default dates from Care Journal
• Demographics Query - add correct drop down list contents to Sex field.
• When opening a folder, all Dependencies were being loaded, not just for that patient.
• The Copy Cells wont highlight more then one cell in Significant events
• New Sig Event: Start date > end date rule is not being re-evaluated when user makes correction. Cannot save & close.
• After upgrade, Access Violation as the software is restarted.
• PrintPreview - all of them need to check screen bounds of the saved settings
• INI file is incorrectly being created in the program folder, instead of in the correct `settings` folder.
• Server Install/upgrade - add a pages about SQL SERVER 2008 R2 and analysis recalcs
• IBID Record Contents Report > Page Setup > Print Preview: shows using horrible Orange toolbar & full screen.
• Care Journal: Print or Page Setup & then clicking Print Preview: window shows full screen and not the dimensions of the parent
• Sig Event: when end date is `too low`, the error created is not 'critical' and so the user can save & close
• Care Journal Help button needs to go to the correct Wiki page
• Validation Messages-> Print-> Preview = Doesn't generate print preview.
• Create Indexes in Server as part of MetaService - but also expose an Index Rebuild public property and assign to client Tools > Database
• Database Information dashbaord item; Validation messages = the Critical count is not initialised in the header label
• [Client 2.2 > Server 1.2] Demographics Query - where is the Sex field?
• Make Max Records available BEFORE running a query !
• Query Performance
• Spelling error (Forcably > Forcibly) on the Server Upgrade.
• Implement index reorg/rebuild option in the user interface
• Initial Action, Disch From Service field values not showing on the card view of the IBID list on the Pt Folder home page
• Pt Folder - refresh not working on IBID list.
• Missing Spark Lines on IBID `front page` (part of Pt Folder)
