Wound Intervention/TX

Used to record any Theatre visits, Sedation or Wound Intervention that has taken place.

To add a theatre visit, sedation or wound intervention click add on the Theatre Wound Intervention Records Toolbar, or the add icon in the centre of the Wound intervention screen. Multiple theatre visits can be added for each dependency record.

Theatre

  • Theatre visit & GA - Tickbox for any theatre visit or General Anesthetic the patient has received that care episode. This forms part of the ibid summary and is used for analysis purposes.
  • Formal Sedation - Tick box for any formal sedation the patient received whilst on the ward, not for theatre visit.
  • Time in Theatre - Free type or use scroll arrows to enter the time the patient entered the operating theatre
  • time Out of Theatre - Free type or use scroll arrows to enter the time the patient came out of the operating theatre.
  • total minutes in Theatre - Automatically fills in the time patient is in the operating theatre, calculated from time input to the In Theatre Out Theatre fields.
  • Donored - Tickbox when patient has received donored skin.
  • Escharotomy - Tickbox when Escharotomy has been performed for cases of full thickness circumferential and near-circumferential skin burns.
  • Debrided TBSA - Free type the total debrided skin surface area.
  • Grafted TBSA - Free type the total grafted Skin surface area.
  • Operator Name - Choose from drop down list or free type the name of the surgeon performing the procedure, the staff members should be added by the system administrator to enable them to appear in the drop down list. For further information regarding adding a new member of staff please see Managing Users.
  • Operator Grade - If the name of the staff member has been added from the drop down list the Operator grade should be automatically filled in, if not choose from drop down list.
  • Operator Code - If the name of the staff member performing the procedure has been added from Operator Name in the drop down list the Operator Code should be automatically filled in, if not free type operator code in to the Operator code field.
  • Supervisor Name - Choose from drop down list or free type the name of the supervisor assisting the surgeon performing the procedure, the staff members should be added by the system administrator to enable them to appear in the drop down list. For further information regarding adding a new member of staff please see Managing Users.
  • Supervisor Grade - If the name of the staff member has been added from the drop down list the Supervisor grade should be automatically filled in, if not choose from drop down list.
  • Supervisor Code - If the name of the staff member performing the procedure has been added from Supervisor Name in the drop down list the Supervisor Code should be automatically filled in, if not free type Supervisor code in to the Supervisor code field.
  • Operative Detail - Free type any useful information or details regarding the procedure.

Consumables

  • Complex Scan - Tickbox when patient has had a complex scsn
  • Chest X-Ray - Tickbox when patient has had a chest x-ray
  • Nursed In Single Cubicle - Tickbox when Patient is nursed in a single cubicle
  • Critical Care Transfer - Tickbox when a critically ill patient has been transferred in order to stabilise them or limit their injuries.
  • Units of Blood (Inc Operative) - Enter the total units of blood used during this procedure
  • Units of FFP (Inc Operative) - Enter the total amount of Fresh Frozen Plasma used during this procedure
  • Units of Platelets (Inc Operative) - Enter the total amount of platelets used during this procedure

Units of HAS (Inc Operative) - Enter the units of Human Serum Albumin used in this procedure

Professional Input times

  • Physcology Input (minutes) - Enter the minutes the psychology department spent assessing the patient
  • Dietician Input (minutes) - Enter the time in minutes the dietician spent assessing the patient
  • SALT Input (minutes)- Enter the SALT (Skin-Associated Lymphoid Tissue) team input time in minutes
  • Ext Medical Review - Enter the time in minutes the patent underwent a medical review
  • Pain Team/Anes Input (minutes) - Enter the time the Pain Team/Anaesthetist input to the patients theatre visit or episode

Once the above fields have been competed, click the save button, the details will show in a summary at the top of the screen. to add further Theatre intervention Details click the "Add" Button
